The Trailair Rota-Flex 5th Wheel Pin Box # LC328330 has a steel sleeve inside of it that those two bolts thread into. If the bolts keep coming loose then the sleeve is damaged and moving around inside the box. Since there isn't a way to access and repair this the only option is a complete replacement of the pin box. If you are measuring only the brake shoe material and not the metal plate and have that 3/16 inch measurement, then it appears the assemblies were either not installed correctly or they did not function as designed.

If your MaxxFan Roof Vent w/ 12V Fan like the part # MA00A04301K that you were looking at suddenly stopped working I recommend using a circuit tester like # PTW2993 to see if power is getting to the fan. Depending on how the fan was installed, it may be attached to a circuit breaker that you can check but most likely it was wired directly to 12V power.
